RENEWAL OF A
REGISTRATION OR LICENSE
Licenses and registrations
expire on December 31 of each year. Forms to renew are mailed to the current
address of each licensee of the Board by December 15. The completed renewal form
needs to be submitted to the Board office with proper fees and proof of
continuing education credit. Veterinarians are required to show proof of 20
hours each year and veterinary technician are required to show proof of 12
hours every two years. Based on year of Registration.
The Veterinary Practice Act (Administrative Code .0106) requires that the Board
office be advised as to all address changes with in 60 days of such changes.
The Board office takes address changes in writing by either mail or fax.
Renewals are due no later
than January 31, of each year. After this date, renewals automatically incur a
late fee of $50.00 which needs to be included at time of submission. Licenses
and registrations are revoked on March 1, of each year.
Renewals will be returned, if the form lacks continuing education credit to renew, not completed, check for wrong amount, etc, this does delay the renewing process.

Please note that name changes require

REQUEST FOR EXTENSION
An Individual who can provide evidence of an incapacitating illness or evidence of other circumstance which constituted a severe and verifiable hardship may request in writing for an extension of time to obtain continuing education. A letter submitted to the Board office for review stating the incapacitating illness or evidence of other severe and verifiable circumstance that to comply with the continuing education requirement would have been impossible or unreasonably burdensome is required. Some extension request may require to be reviewed by the full Board.
The written request for an
extension to satisfy the continuing education requirement should be submitted
to the Board office as quickly as possible. Some request may require to be
reviewed by the full Board, that review will take place during the next scheduled
meeting of the Board. Not all requests for extension of time are granted.
REINSTATEMENT OF A
REGISTARTION OR LICENSE
If an individual veterinary license or veterinary technician registration has a status of revoked with the Board and needs to have the license or registration reinstated. Contact the Board office, submit a reinstatement form, renewal fees, late penalty for each year, reinstatement fee of $25.00, and continuing education credit for each year in which the license/registration was revoked.
When a veterinary license is reinstated is it current until December 31 of the year that the license is reinstated.
When a veterinary technician registration is reinstated, it will be current for that two year renewal period.
Individuals that are reinstated will need continuing education credits for the upcoming renewal cycle.

INACTIVE STATUS OF A
VETERINARY LICENSE
A Veterinarian in good standing with the Board, who has ceased the practice of veterinary medicine, may according to Administrative Code .0309 request inactive status. The request may be granted if the veterinarian has ceased the practice of veterinary medicine. Meaning the individual is not working in a capacity that requires a veterinary license or degree in veterinary medicine, and is no longer providing veterinary services.
The request needs to be in writing to the Board office. Individual still practicing/working in veterinary medicine, In work where a degree in veterinary medicine is required, preferred, ect. would not be eligible for inactive status.
An individual who has an inactive status may have the license reinstated at any time. The requirement to reinstate is proof of the current year continuing education credit, renewal fee and reinstatement application. Contact the Board office for more information.
There is no inactive status for veterinary technicians. It is required to maintain an active registration with the Board to use the title registered veterinary technician, veterinary technician, technician, ect.
