CheckREQUEST:  Verification of a License/Registration or to Request a “Letter of Good Standing”
Fee: $12.00 per State
Your check should be payable to NCVMB (North Carolina Veterinary Medical Board). The estimated turn-around time for a verification request is within 48 hours once received.

Request needs to be submitted in writing to the Board office and should contain:
    1.) The state(s) where the verification is to be sent
    2.) Your current address
    3.) A current phone number

Once the request has been processed, you will be mailed a copy of the license/registration verification. This copy is for your records and serves as a notification of when the official verification was mailed.


Pharmacies, banks or other entities seeking confirmation of North Carolina licensure of veterinarians or registration of veterinary technicians can contact the Board office at (919) 854-5601.

CheckREQUEST:  Disciplinary Action Information

Some state agency web sites provide a listing of licensees or registrants who have disciplinary actions. The North Carolina Veterinary Medical Board realizes that listings of this nature do not give a full understanding of the situation that caused the disciplinary action. Therefore, the NC Board chooses not to provide such information on this web site.

The Board will provide disciplinary information on any licensee or registrant upon written request. The request must be made in writing and mailed, faxed or e-mailed to the Board office. The Board's response will most likely be by return mail and contain the letter of dismissal or final disciplinary action. This is provided so that the consumer can make an informed decision regarding their choice of veterinary care. Please provide your name, mailing address, telephone number and whom you are requesting information on.

When requesting disciplinary information, the request must pertain to an individual and not a veterinary practice facility. The Board does not take complaints against veterinary practice facilities, complaints are against individuals. This assures that an individual cannot clear their record by moving to another veterinary practice.

CheckREQUEST:  Practice Facility Inspection Information
Fee: No Charge
The Board will provide inspection information on any facility upon written request. The request must be made in writing and mailed, faxed or e-mailed to the Board office. The Board's response will most likely be by return mail and contain information on the most current inspection. This is provided so that the consumer can make an informed decision regarding their choice of a veterinary professional.

Inspections occur every two (2) years. A new practice is inspected prior to opening and again the following year, then every two (2) years thereafter.

CheckREQUEST:  Name Change

Fee: No Charge
In order for the Board to change your name in our system, you will be required to supply a copy of a legal document (e.g. marriage certificate, divorce paper) and a copy of your Social Security card reflecting your new name. In addition, be sure to notify us of any address change.
